The MCNDH Diabetes Prevention and Management Program works with clinicians and community resources to provide tools and education to those who have diabetes or are at risk for the disease. Five MCNDH clinics have a Program team on-site to assist patients.

If you are interested in attending any of the prevention or management programs, you may ask your provider to refer you, or you may also call your nearest participating clinic and ask for the Diabetes Program directly.
